The fee is published here so you have it before you call. A conversation about whether an evaluation is the right thing is a better use of fifteen minutes than a conversation about what it costs.
This covers the full evaluation:
A $1,000 deposit confirms the appointment. It is applied to the fee, not added to it.
The remaining balance is due before the report is released.
Cancellations with at least three business days notice are refunded in full.
Cancellations with less than three business days notice, and appointments that are not attended, forfeit the deposit.
One reschedule with notice costs nothing.
This is a private pay practice. Insurance is not billed.
An evaluation that is not scoped or limited by an insurer covers what the referral question actually requires, and produces documentation clean enough for outside reviewers to accept.
Preparation begins as soon as an evaluation is scheduled, and one part of it takes several weeks to assemble. See Preparing For Your Evaluation.
You have the right to receive an estimate explaining how much your care will cost.
Under federal law, health care providers must give patients who do not have insurance, or who are not using insurance, an estimate of the expected charges for medical items and services.
You have the right to receive a good faith estimate for the total expected cost of any non-emergency items or services. This includes related costs such as testing, scoring, and report preparation.
You can ask any health care provider for a good faith estimate before you schedule an item or service.
If you schedule an item or service at least 3 business days in advance, your provider must give you a good faith estimate in writing within 1 business day of scheduling. You can also ask for one before you schedule.
If you receive a bill that is at least $400 more than your good faith estimate, you can dispute the bill.
Make sure to save a copy or a picture of your good faith estimate.
